Introduction
The number of people who are living with diabetes in the US has increased by almost 50% and this totals up to 29 million Americans. In the whole world, diabetes has affected more than 380 million people all over the world and according to a study done by the world health organization, the number are expected to double by 2030 in the world. Living with diabetes has a lot of emotional impacts to the patients as well as physical effects. The disease is costly in terms of treatment and management. It is estimated that the annual amount of money which is used by the US citizens is more than $245 billion in the management of diabetes. Researching this topic concerning diabetes has been spearheaded by the danger which the disease has caused to the American people and it is aimed towards providing an effective care plan which is meant to help the participants in the society.
According to the healthy people 2020 goals in the society, there are several objectives which are expected to help the diabetic people in the society. The health people 2020 goals in diabetes care are meant to help in reducing the death rates of the diabetes patients through providing effective services, they are aimed towards attaining health equity in the society and assist in the reduction of disparities in the society in terms of diabetes care, they help in creation of social and physical environments which assist in promotion of good health for everyone and they assist in the promotion of quality life and increasing the development of health to people in all stages of life (Model, 2015). The reason why it is important to study about the healthy people 2020 is because it helps in understanding the goals which are set concerning the provision of care of the diabetic people in the US and it also helps to understand the importance of care plans in meeting those goals.

From the assessment data, it is found out that the patients did not observe all the requirements of diabetes care in terms of eating unhealthy foods, smoking and drinking alcohol, lack of checking the glucose levels and lack of doing the regular exercises.

Mr. AB was a diabetic patient and through an interview, it was noticed that he was not following the correct procedure which is required in order to assist in fighting against diabetes (Association, 2014). This is because the nurse did the interview and found out that he was not doing the necessary physical activities, he was not eating healthy foods and also he was not checking his glucose levels in the body because he did not care.
Desired outcomes
Identification all the risk factors which might be leading to the unstable levels of glucose have to be done. The risk factors which are associated with lack of physical activities is also done as well as the risk factors which are associated with eating unhealthy foods is also done. This is done through verbalization of the importance of balancing the body of the diabetic patient with energy needs in the body. The identified risk factor are also handled effectively in order to help in the stabilization of the patient. In doing physical activities, the risk factors are verbalized to the patient in order to make him aware of the problems which he might have brought to himself.

The evaluation criteria which is followed in the case of the diabetic patient involves a well thought out procedure which shows the effects of lack of taking the glucose measurements , effects of taking unhealthy foods , drinking alcohol and smoking as well as the effects of not doing physical activities on the emotions of the patients (Funnell, 2009). This evaluation is meant to assist in identifying the main measures which have to be used on the patient in order to bring him back on course.

Importance of the weekly assignments on creation of a care plan
In the assignment where there was a case study concerning diabetes control in relation to Mr. AB, an appropriate care plan can be made from the interview. The assignment was effective in the creation of a well-managed care plan because it elaborates the problems which affected Mr. AB including lack of proper eating habits, lack of monitoring the glucose levels, lack of doing physical activities and getting involved in drinking alcohol and smoking. This was perfect in the creation of a perfect care plan for the patients because the problems which were presented could lead to increased resistance of insulin intake in the body. A care plan was supposed to be established in order to assist in the management of the patient better.
In the week whereby there was a study concerning the general effect of diabetes to the nation and assessment criterion of the diabetes patients, the general outcome of the assignment showed that carrying a full assessment of the diabetic patient can help in recognizing some of the problems which may face the diabetic patients. This assessment was a way of recognizing the problems which come as a result of lack of proper care of the diabetic patients. The numbers show that diabetes has claimed the lives of many people and therefore an effective plan has to be made.
In the evaluation of the national resources which are used in diabetes care, it was noticed that the resources are expensive in terms of fees which are used. The integration of the resources with the diabetes care plans is very important because it can assist in managing the disease. However, it requires trained staff and a lot of finances. The resources help in providing quality services to the diabetic patients and therefore they have to be used in the creation of an effective care plan.
In addressing the support needs of most of the diabetic patients, appropriate interventions to the caregivers have to be taken. The nurses usually play a very important role in supporting the patients’ needs and treatment of the condition. Therefore it is important to consider appropriate measures which can be used in the management of the diabetic patients. The environment and the social determinants can affect the needs of the diabetic patients and this may affect the overall effectiveness of the created plans which are used in managing the patients.
